Specialist, Prevention Outreach

*** This position will be based in Mentor with regular travel to our Ashtabula, Painesville, and Willoughby locations. ***
 
SCOPE OF ROLE
Reporting to the Manager, HIV Program the Specialist, Prevention Outreach is responsible for providing quality and comprehensive Prevention services (HIV, viral hepatitis, sexually transmitted infections (STIs), and other public health issues as needed, including testing, education, risk reduction, screening). The Outreach Specialist will collaborate across the agency and assist in the identification of new testing opportunities, while engaging priority populations. 
 
HOW YOU’LL SUCCEED ____________________________________________________________________
  • Provides client-centered prevention counseling, testing, and referral (CTR) for HIV, viral hepatitis, and sexually transmitted infections.
  • Assesses, identifies needs, and provides connection to services/care for clients and community members, including linkage to Pre-Exposure Prophylaxi (PrEP) and HIV care.
  • Provides PrEP education, linkage to PrEP navigation and care services, and provides adherence support for Signature Health patients currently prescribed PrEP.
  • Distributes condoms throughout agency and community.
  • Participates in outreach strategies in various environments (in healthcare settings, community, partner agencies, street-based, and community organizations.)
  • Provides Prevention education and training both within the agency and externally to other agencies, community organizations, and individuals in the community.
  • Provides services at community events and health fairs, including CTR services, education, and set-up/tear-down.
  • Complies with all program protocols, including those regarding confidentiality of patient information. 
  • Completes and reviews CTR paperwork, enters data into centralized database, and assists with submission of program reports as directed.  
  • Communicates with public health agencies as directed. 
  • Participates in evaluative and quality assurance activities.
  • Maintains professional accountability to the agency, funders, and clients served. 
  • Assists Program Manager in updating program plan and program goals.
  • Collaborates with care teams across Signature Health, including Ryan White and Infectious Disease services, Primary Care, Family Planning, behavioral health services, substance use and recovery services, and case management.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform remote services as determined by Signature Health.
  • Adherence and completion of compliance training provided by Signature Health.
  • Responsible for and completes all productivity/documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Other duties as assigned.
 
KNOWLEDGE & EXPERIENCE ______________________________________________________________
  • High school diploma or equivalent required.
  • Associates Degree in Human Services or related field preferred.
  • Experience working with HIV, LGBTQ+ communities, and/or substance use disorder, including CTR certification, CDCA, etc., is preferred.
  • Must have knowledge, awareness, sensitivity, comfort, and competence regarding implicit bias, cultural competence and humility, health disparities, LGBTQ+ communities, people of color, substance use disorders, sexual health, and trauma informed care.
  • Ability to work in environment that promotes compassion, service, and social justice.
  • Ability to compassionately have difficult conversations and de-escalate situations with clients.

WORKING CONDITIONS ___________________________________________________________________
  • Work is normally performed in a typical interior/office/clinical work environment.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Requires periods of sitting, standing, telephone, and computer work.
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
