Community Health Worker- NME Manager

Job Summary:
The Community Health Worker Manager will manage a team of CHW’s that locate, engage, and educate members referred to AbsoluteCare by their health plan to have the member switch their PCP to AbsoluteCare or to have them enroll in other services with AbsoluteCare.  You will establish trust and build meaningful relationships with members. This position will lead a large team of Community Health Workers in making an impact in the day-to-day life of the members AbsoluteCare serves – those most in need of concierge care. Providing support and coaching to the CHW’s to meet program outcomes. This role will monitor the overall performance of the team, utilizing Key Performance Indicators to help track progress, build on success, and pinpoint areas for improvement.  Instill and promote AbsoluteCare’s 5-star customers service philosophy in the medical center and in the community.

Duties:
  • Successfully manage a team of CHW’s to meet program outcomes
  • Execute creative outreach strategies that will:

  • Be in the community as the “feet on the street” to locate and engage members from a targeted list, who might be hard to locate or build rapport with.
  • Attract potential members to enroll at AbsoluteCare as their PCP or in ancillary services such as Care Management or Behavioral Health services.
  • Effectively explain the benefits of enrolling at AbsoluteCare.
  • Assign members to CHW based on geography, and case size
  • Perform telephonic or home outreach to targeted list which includes “cold calling” to assigned members in attempt to have member choose AbsoluteCare as their new PCP.
  • Provide daily support to CHW team which includes scheduled 1:1’s
 Education and Experience
  • Must have a valid driver’s license and reliable personal vehicle.
  • Previous experience managing a team.
  • Proficient in PC skills, such as Microsoft Office.
  • High school diploma or equivalent required, Bachelor’s degree preferred.
  • Community Health Worker experience/certification desirable.
  • Ability to work evening and weekend hours when needed.
